OnabotulinumtoxinA, with a close look at regional labels and the science of facial feedback.

Scope of this monograph#

Clinical summary: BOTOX Cosmetic, US. VISTABEL and Australian BOTOX are separate regional records.

Clinical statements below summarize the attached documents. They are not a complete prescribing label. Match the exact local product and current instructions to the clinical decision.

Composition & formulation#

OnabotulinumtoxinA in a dried formulation for reconstitution. The US label lists 50- and 100-unit single-dose vials.[2]

How it works#

Inhibits acetylcholine release at cholinergic nerve terminals, temporarily reducing activity in the treated muscles.[2]

Indications & treatment areas#

The US label covers moderate-to-severe glabellar, lateral canthal and forehead lines, and platysma bands in adults. These indications cannot be transferred to VISTABEL.[2][3]

Application considerations#

Intramuscular use requires anatomical assessment and product-specific preparation. Potency units cannot be converted to those of another toxin.[2]

Contraindications & precautions#

Contraindicated with relevant hypersensitivity or infection at the injection site. Neuromuscular disease, dysphagia and interacting medicines require particular assessment.[2]

Adverse effects & safety#

Ptosis, headache and local reactions occur. Distant toxin spread can impair swallowing or breathing and requires urgent medical attention.[2]

Duration & follow-up#

Effects are temporary and depend on the treated indication and endpoint. Consult the indication-specific clinical-study section rather than a single family-wide duration.[2]

Distinctions that matter#

Facial-feedback research studies neural activity, not an approved psychiatric treatment indication. A mechanistic finding must not become a therapeutic promise.[5][2]

Four markets. Separate evidence.#

Documents below identify what has been attached in this research edition. “Source needed” means unverified here; it is not a statement that a product is unapproved.

German product information for VISTABEL; no equivalence of indications is inferred.[3]

UK SmPC updated May 2026 includes facial-line uses and platysma prominence. This is a separate regional presentation from US BOTOX Cosmetic.[1]

US label includes platysma bands following its October 2024 update.[2]

ARTG 67311 identifies the Australian presentation; the local PI must be used for prescribing.[4]
